Member Application Form

Eligibility for Membership
* Members must be 18 or over.
* Pay Membership Fees.
* Complete Membership Forms.
    - Dues & Responsibilities, Member Release Form, General Use and Safety Policy Form
* Complete Basic Training.

Member Privileges
* Access to the physical workspace and equipment (Staff will set hours and coordinate access to equipment).
* Key card to the physical workspace. Membership begins on day key card is provided.
* Free safety training and equipment use training of power tools and equipment prior to use.
* Free or discounted entry to workshops, classes, or MAKERSPACE activities.

Member Responsibilities
* Members must follow all general use and safety policies of the Brookings Area MAKERSPACE.
* Members must pay the membership fee.
* At the time a member's eligibility expires, the member must return his or her key. There will be a $45 fee assessed for anyone who does not return their key.

Resignation and Termination
Any member may resign their membership by submitting a written resignation with Brookings Economic Development Corporation and returning their assigned key card. Resignation shall not relieve a member of unpaid dues, or other charges previously accrued. A member can have their membership terminated if they do not abide by the rules.

Tools and Equipment
Safety and equipment training is required for membership. As a member, I understand that if I operate any tool in an unsafe manner or if any of my actions jeopardizes my safety or safety of others, my membership in the Brookings Area MAKERSPACE may be revoked.

Members are also required to read the GENERAL USE AND SAFETY POLICY and sign the Member Release Form, which will be provided when you receive your key card.

Title VI of the Civil Rights Act of 1964 requires “Race and Ethnic” data collection from beneficiaries of federally assisted programs. Please note “Disclosure Clause” below:
